The School Administration has instituted a Medical Report that is designed to provide all the necessary information related to your children’s health. This will allow us to provide informed care and follow up on any medical condition or emergency, concerning your children while at school. This form is obligatory and will be kept confidential in the Medical Department.

Please print out the Medical Report and return the completed form signed by your family physician to school with your children on the first day of school in a sealed envelope marked “Medical Report”.
