Dear Parents,

In the aim of protecting the health of your son/daughter, the School Administration is sending the Medical Report Form attached with this circular.

You are kindly asked to indicate any changes that have occurred in your son’s/daughter’s health from October 2021 to September 2022. This form is obligatory for all students and will be kept confidential in the Medical Department.

Please print out the Medical Report Update and return it filled and signed on the first day of school with your child in a sealed envelope marked “Medical Report Update”.
